Quality Management
The Yorkshire and Humber Deanery is fully committed to the principle of ensuring the highest quality of Postgraduate Medical and Dental training. In putting this into practice we apply recognised methodologies as set down by the relevant governing bodies, principally GMC.
Quality Management Manual
Our Quality Management Manual makes explicit our responsibilities for quality management, including the quality control requirements for Local Education Providers (LEP).
The main manual and it's appendices are available to download from the panel on the right.
It is intended to be a reference tool and will be updated on a periodic basis. Users are encouraged to feedback on its contents to emma.jones@yh.hee.nhs.uk
Annual Reports and Visiting
Each year Trusts and Schools are required to complete a self assessment form, providing evidence that training provided meets the standards of the GMC, and where there are exceptions to those standards the actions taken or planned. The evidence is mapped to both the GMC Generic Standards for Training and the Deanery's Educational Standards. The Trust Postgraduate Medical Education Report will be reviewed by the Deanery prior to the Annual trust Visit and form the basis of discussion with the Chief Executive (or representative) of the Trust.
The self assessment reports are based on the National Association of Clinical Tutors (NACT) reporting template and around the GMC domains.
The Annual Reports will enable the Deanery to:
- monitor performance against standards
- identify and review quality control systems at the trust and Local Faculty Group level
- identify and review quality improvement priorities and plans
- identify and share notable practice across the Deanery

The Regulator (GMC)
GMC (the General Medical Council) is the independent statutory body, responsible for overseeing and promoting the development of postgraduate medical education and training for all specialties, including general practice, across the UK.
GMC's responsibilities include:
- Establishing standards and requirements for postgraduate medical education and training.
- Making sure these standards and requirements are met.
- Developing and promoting postgraduate medical education and training across the country.
The GMC have published a new framework for their work to improve quality in medical training Quality Improvement Framework (QIF).
Deanery Visits
The GMC is responsible by law for the approval of postgraduate medical education and training in all medical specialties, including general practice. The GMC will quality assure postgraduate medical education and training in a number of ways but central to their quality assurance processes will be a Deanery-wide, cross-speciality visits programme.
Curricula Approvals
All Royal Colleges and Faculties must submit their Completion of Training (CCT) specialty curricula to be considered against GMC's Standards for Curricula. All information about the approval of Curricula, a summary of the curricula submitted and more is available on the GMC website.
GMC Deanery Wide Visits
Quality Assurance of the Foundation Programme (QAFP) visit June 2010
In June 2010 the GMC visited the Yorkshire and the Humber Deanery and it's three Foundation Schools. A copy of the final report can be found on the GMC's website.
GMC (Formerly PMETB) Deanery-wide Visit 10-12 March 2009
In March 2009 GMC visited the newly established Yorkshire and the Humber Deanery (formally the South Yorkshire and South Humber and the Yorkshire Deaneries).
As part of the process the GMC visiting panel reviewed the quality of training at the following sites: Huddersfield Royal Infirmary, Hull Royal Infirmary, Northern General Hospital and St James University and in the following specialties: Health Care of the Elderly, Core Medicine, General Internal Medicine and ENT.

Approval Process for the establishment or disestablishment of training grade posts
The General Medical Council (GMC) is the sole competent authority responsible for the approval of posts and programmes that directly contribute to the award of a CCT. Trainee doctors who wish to receive a CCT at completion of their training must follow a GMC approved curricula in GMC approved training posts and programmes.
Using the Generic standards for training, GMC assess each application and grant conditional or unconditional approval, taking into account the views of the Deanery and the college or faculty.
Approval of a Specialty Training Programme (Hospital and GP)
To receive approval of a training programme Heads of Schools with their local Programme Directors complete and submit a GMC Form A to the Deanery. All programmes within the Yorkshire and the Humber Deanery have undertaken this process and received formal approval by the GMC. Approval for the establishment of a new Training Post (Hospital and GP)
To establish or disestablish a post (i.e. a change to a GMC approved programme) , a Head of School and or Training Programme Programme must first seek the Deanery's approval using the Deanery's Establishment Control Form or Dis-establishment Control Form. The application will be considered by the Postgraduate Dean.
Once a signed and approved Establishment Control Form has been received and prior to any recruitment taking place the GMC must approve the change to the programme.
To gain GMC approval the following paperwork should then be submitted to the Deanery: an amended Form A, a covering letter outlining the reason for the changes and a completed GMC form B or form B (GP) detailing the details of the new post and confirmation that the relevant Royal College has approved the changes of the programme.
NOTE: GMC will not retrospectively approve posts or programmes - there are no exceptions.
Why a FORM B?
The Deanery will require a completed FORM B for each additional post applied for. This is to ensure that Programme Directors are able to identify each of the posts (ST posts, FTSTA posts and LTFT posts) within the approved programme for future monitoring and quality assurance purposes.

Deanery End of Placement Questionnaire
We recognise the value of trainees' opinions. An End of Placement Feedback Questionnaire (EoPFQ) has been designed to capture the views of trainees about postgraduate medical education training posts that they have occupied in the Deanery. Core questions are mapped to the GMC quality domains, and towards the end of 2012 there will be a maximum of 17 questions per specialty. All Foundation, Specialty and General Practice trainees are expected to complete this local questionnaire at the end of each placement. Data is summarised on a yearly basis and reported back to Heads of Schools and Training Programme Directors.
Data from the National Training Surveys is also used to triangulate against findings from the local questionnaire.

National Training Surveys
GMC Trainee Survey 2013
The National Survey of Trainee Doctors provides a national picture of trainees' perceptions of their training posts and gives GMC and Deaneries invaluable and direct information to help shape the future of postgraduate medical education and training in the UK. The results from the national survey are used throughout the year to inform our quality management work and we hope that trainees and trainers have seen improvements as a result.
In 2013 the trainee survey opens nationally on 26th March.
You can complete the National Training Survey through your GMC Online account. You do not need to wait for an invitation email; you can simply log on and complete it any time between 26 March and noon on 8 May.
The survey asks trainees to respond regarding the post they were in on 26th March 2013. All trainees will have six weeks to complete their survey.
In the past there have been issues with some email providers automatically deleting survey invitation emails as spam. Therefore we recommend that trainees check their spam folders regularly during these months.
GMC state that completion of the survey is mandatory. On completion of the survey trainees will be issued a confirmation code on-screen. Trainees will be able to view and print this code whenever they log into GMC Online. This can then be used as evidence of participation if this is requested at a RITA, ARCP or other meetings.

2012 Results
Results of the 2012 survey are now available in the public area of the GMC Reporting website.
The Deanery is working on analysing the results which will be sent to the Local Education Providers and Heads of School in the next few weeks. You can also see results and reports from previous surveys in the GMC's Survey Results page.
Support
The GMC have set up dedicated support pages for all doctors participating in the surveys. This includes a frequently asked questions section for both the deaneries and all trainees participating in the survey. These can be found on the support pages.
