East

Welcome to the East rotation.

The North & East Yorkshire Programme, otherwise known as 'The East' offers excellent training and prides itself on supporting trainees well throughout their training.  This is a great part of the world to live!  York with its backdrop of Viking and Roman history and wealth of cobbled streets and tea rooms. Scarborough and Grimsby offer the seaside and the Humber estuary takes you up to Hull (City of Culture 2019) and Scunthorpe where a visit to The Deep or old harbor pubs will surprise you. Between the hospitals is the beautiful countryside of the Yorkshire Wolds or the North Yorkshire Moors. Whether you want to cycle in the Wolds, walk in the heather moors or eat fish and chips in Whitby we can guarantee you some great days out!

In addition, the Specialist (Higher or ST) Anaesthesia rotation includes the above 5 hospitals, plus:

  • Leeds Teaching Hospitals 
  • Mid Yorks Teaching Hospitals 

There are also several smaller sites which fall within these Trusts, at which trainees may be required to complete placements as part of their training programme in the East.

Our region covers a large area and we aim to accommodate trainees’ placement requests as much as possible. All rotations will involve some commuting, unless trainees plan to move house with their rotations. Generally training will be structured as follows;

 

Rotations

Rotations take place every 6 months in February and August on the first Wednesday of the month.

CTs: CTs will spend their 2 years in two different hospitals. Usually this will consist of one year in a smaller  hospital and the other in a larger one.

ST3-7 (recruited from Aug 18): The structure of training in HEE YH changed in 2018. As a result, trainees should be aware of the first four years of their ST rotation. Further information about this change is available in the Rotation changes and FAQ section.

 

Advanced training opportunites available in the East Rotation

We work to ensure that all trainees have access (sometimes by competitive interview) to all types of Advanced training. Within Health Education England - Yorkshire and the Humber, there are posts for advanced training in ICM (changing with single CCT ICM), Paediatrics and Pain. Sometimes this will involve rotating to other hospitals within region, such as Sheffield or Leeds. We can also accommodate less well defined specialisation and had trainees who have undertaken higher training in Hepatobiliary and Management over the last few years. We have an ongoing Clinical Skills Fellow post for those with an interest in education.

Your Training Programme Directors will do their best to accommodate requests and avoid unwieldy commutes.

 

Exams
Regional exam courses (for East and West trainees) run throughout the year and trainees are expected to attend. Dove-tailed with these are various Simulation Training Days in York and Hull which all trainees are able to attend. Exam focused teaching takes place on occasional Saturday mornings throughout the year, and before each oral exam, a very popular preparation day takes place. On top of this, each hospital runs some in-house teaching. We are proud that, as a locality, we provide a large amount of teaching from a huge number of very enthusiastic trainers, and we are pleased to see our exam success reflecting this.

Simulation

Hull has a state of the art simulation centre (the Clinical Skills Facility) which is a beautiful building, where you will all be trained on a simulator throughout your training. York also has a simulated anaesthetic room. In Yorkshire we are ahead of many other localities with our simulation facilities, a coordinated and structured programme is now in place to ensure we make the most of this. We have 'Associate Simulations Fellow' posts, for trainees to apply, which helps with their training as an educator, whilst providing training for the rest of the region.

Hull Royal Infirmary (HRI)

HRI is a tertiary centre with the friendliness of a district general. It is a major trauma centre for large swathes of East and North Yorkshire and North Lincolnshire and is due to have an on-site helipad 2018.

In total there are 20 modern theatres offering ample opportunities for airway experience as well as varied emergency and elective work. There is also a small separate day surgery unit. HEYHT has a total of 63 consultants across the trust with many different specialist interests. The department has two primary examiners.

Trainees are well supported by senior staff including excellent exam preparation and regular in-house exam courses. There are also regular teaching sessions, tailored to experience, aimed at developing skills and knowledge, enabling progression as an anaesthetist. 

The Final FRCA teaching programme is led by Dr Sarah Price.

Monthly audit meetings provide plentiful opportunity to become involved and present in a friendly atmosphere. There is also a regular trainee journal club.

Two mixed High Dependency & Intensive Care Units provide varied experience in dealing with critically ill medical and surgical patients, with up to 24 beds in total. This is complimented by keen consultant staff and ACCPs who enable trainees to have protected teaching time.

The adjoined Women's and Children's Hospital provides obstetric services with approximately 6000 deliveries per year and as a tertiary referral centre gets interesting/challenging cases to develop skills.

 

Castle Hill Hospital (CHH)

CHH is located in the North West of the city, and is the more attractive of the two hospitals, sat above Cottingham, a picturesque village originating from the 11th century. It is close to the pretty town of Beverley and is an excellent base to explore the local attractions surrounding Hull.

The hospital itself has been there 100 years but was recently modernised and now includes the Queen's Centre for Oncology and Haematology.

We work closely with Hull and East Yorkshire Hospitals in managing vascular, cardiac, cardiothoracic and neurosurgical patients and work with EMBRACE for the management of paediatric critically ill patients. We also have close links with Hull and East Yorkshire Medical School.

The anaesthetic department provides services for eight operational theatres covering general surgery, gynaecology, urology, elective and emergency orthopaedics, ENT, maxillofacial, breast paediatrics and ophthalmic surgery. There is an additional theatre for Maternity.

Additional supervised training opportunities are available in remote sites including cardiac catheterisation suites and radiology units

Obstetric anaesthesia includes round the clock epidural service. There are approximately 2700 deliveries annually

Critical care currently has six ITU and 7 HDU beds. The ITU is supported by extended Consultant cover from 08;00 AM to 21:00 PM and additional HDU cover from 08:00 AM to 13:00PM.Last year ITU had 415 admissions and HDU had 752 admissions.

There are opportunities available for advanced trainees to acquire expertise in lung ultra-sound and Focussed Echocardiography.

Anaesthetic trainees work an EWTD compliant 1:8 rota and there are plenty of opportunities for quality improvement, medical student teaching and opportunity to present at various in house teaching programmes.

Excellent opportunities for professional development include

1. Involvement as faculty for regional and RCOA approved teaching courses.
2. Involvement in multi-centre research projects and publication of national and international posters
3. Work towards the development of an in situ simulation programme.

The Anaesthesia Department

We are a team of 16 consultants, 1 associate specialist, 6 staff grade and 4 trust grade doctors, and trainees on rotation from the East School of Anaesthesia.

The department provides anaesthetic services to a range of specialties at Scunthorpe General Hospital and Goole and District Hospital, including General Surgery, Colo-rectal Surgery, ENT, Ophthalmology, Urology, Trauma and Orthopaedics, Obstetrics, Gynaecology, Oral and Maxillofacial surgery, and anaesthesia at remote sites (e.g. cardioversion, radiology procedures etc.). The department also runs chronic pain clinics and a pre-assessment clinics.

The department at Scunthorpe is situated on Floor E, opposite the entrance to the intensive care unit and adjacent to the Neonatal Intensive Care Unit. Main theatres and central delivery Suite are also located on the same floor. There are 8 theatres in Scunthorpe and 2 in Goole. ICU consists of 8 beds.

Teaching and Training

We aim to provide trainees with an enriching and enjoyable learning experience in the anaesthetic department at Scunthorpe General Hospital. Trainees gain excellent experience at all levels of training from a wide variety of elective and emergency cases.

Trainees take part in a EWTD compliant 1 in 8 on call rota.

There are consultants in the department who are very enthusiastic and passionate about teaching. There are also consultants who teach on national and international courses. We organise joint teaching programme for CT and ACCS trainees in anaesthesia along with Hull and Grimsby hospitals. There is also a lunchtime teaching programme- all doctors in the department contribute to the teaching. Trainees are encouraged to attend teaching sessions and courses offered by the school of anaesthesia, and to make full use of their study leave. There is also opportunity to learn on the pain ward rounds.

The department provides excellent viva practice to trainees appearing for the primary as well as final FRCA exams.

  1. Core Level: all modules
  2. Intermediate Level: all modules except Cardiac anaesthesia and neuro-anaesthesia
  3. Higher Level: All general duties modules, ICM.
  4. Senior trainees will have the opportunity to complete management module (Annex G)

There are excellent opportunities at all levels to get involved in teaching, audits, quality improvement projects and poster presentations.

General Information

Scunthorpe in located in the North Lincolnshire district, which is a mixed industrial and rural community and has a population of about 200000. Scunthorpe is a busy industrial town. The redeveloped town centre and extensive parklands and woodlands offer a pleasant living and working environment. Brigg and Barton are small market towns concerned with light industries and agriculture. Goole is located in the East Riding of Yorkshire.

Scunthorpe General Hospital is a part of the Northern Lincolnshire and Goole (NLaG) NHS Foundation Trust along with Goole and District Hospital, and Diana, Princess of Wales Hospital in Grimsby. Travel

Scunthorpe and Goole are directly linked by motorway- Sheffield, Doncaster, Leeds, York and Hull are all less than an hour’s drive, as is Lincoln on the A15. The Humber Bridge gives easy access to the city of Hull.

Rail services from Scunthorpe connect via Doncaster with the main intercity service to London, and to the north of the country. There is also a direct train to Manchester and Manchester airport.

Humberside Airport is situated within the district and offers holiday flights and world-wide connections via Amsterdam Airport. The Robin Hood Airport near Doncaster offers regular flights to various destinations.

Scarborough and Bridlington Hospitals became part of York Teaching Hospitals NHS Foundation Trust in 2012 which serves approximately 530,000 people in an area of 3,400 square miles.  The eastern part of this area covers a diverse population from the towns of Scarborough and Bridlington, approaching 100,000 and 40,000 residents respectively, to the rural communities of the North Yorkshire moors and Yorkshire Wolds. The region includes beautiful countryside and coastal resorts which attract large tourist numbers leading to a doubling of the population in the summer months. Although geographically distant from other hospitals in the region, commuting is usually pleasant (with barn owls to see) and car parking easy. Our location also enhances training opportunities as distance from tertiary centres often means unselected emergency admissions despite having Trauma Unit status.

The anaesthetic department provides services for general surgery, gynaecology, urology, elective and emergency orthopaedics, ENT, maxillofacial, breast and ophthalmic surgery. The Intensive Care Unit currently has capacity for up to five level three patients with plans to expand to an additional bed. We deliver ICM training at basic level and support a full time outreach service.  Obstetric anaesthesia is provided 24 hours per day to delivery suite which delivers 1,600 babies per year. Whilst predominantly based in Scarborough, there is ongoing expansion in Bridlington where elective orthopaedic surgery is now based using an established and internationally recognised enhanced recovery programme. Plans are in progress to develop this site further increasing to five theatres and larger numbers of short stay procedures.

The department currently has 15 consultants with further recruitment and expansion in progress. We have a diverse range of interests including ICM, trauma, acute and chronic pain management, regional anaesthesia and education with very strong links to the Hull York medical school. We work closely with our colleagues in York, both clinically and educationally, and also with other anaesthetic departments and hospitals in the region.

Anaesthetic trainees work an EWTD compliant 1:8 rota and there are plenty of opportunities for quality improvement, medical student teaching and social activities. We have weekly in-house anaesthetic and ICM teaching, share monthly all-day teaching with our colleagues in York and have access to regional exam-specific teaching.  Novice anaesthetists benefit from no out of hours work for the initial three months, leading to rapid acquisition of the initial competencies, and regional specific teaching sessions.

University Teaching Hospital 

York Teaching Hospital NHS Foundation Trust was granted its license as a Foundation Trust in April 2007 and has been a University Teaching hospital as part of the Hull and York Medical School (HYMS) since 2003. 

In July 2012 York merged with Scarborough and North East Yorkshire Healthcare NHS Trust, bringing York, Scarborough and Bridlington Hospitals into one Trust.

As a trainee at York Hospital you will only be expected to work in York. 

Anaesthetic Department

We are a department of 45 Consultants, 6 staff grades and 16 trainees (of all levels of training). We aim to be supportive clinically and educationally demonstrated by a Consultant anaesthetic presence in the hospital until midnight (and overnight if trainee skill mix requires).

Clinical Experience
 

Anaesthesia

We offer a wide range of specialities required for basic, intermediate and higher levels of training with the exception of neurosurgery, cardiothoracic and higher paediatrics.

At the advanced level of training we offer training in Chronic pain, Obstetrics, Perioperative Medicine, Regional and Orthopaedics and Vascular Surgery.

 

Theatres: We have 11 operational main theatres and an integrated theatre and Vascular Imaging Unit currently under construction within the theatre complex. In addition to this there are 6 day theatres, 2 ophthalmic and 2 theatres to which we are seconded at an Independent Treatment Centre (purely cold Orthopaedics).

Surgical specialities undertaken in York include: General, Bariatric, Vascular, Orthopaedics, Plastics, Breast, Trauma, Urology, Gynaecology, Maxillo-facial, ENT and Ophthalmology.

Perioperative Medicine: York has led the way in perioperative medicine. This began with in the introduction of cardiopulmonary exercise testing in 2004 to risk stratify patients, optimism and provide goal directed therapy to improve outcome.. The perioperative anaesthetic care has been extended further, with the development of daily Consultant Anaesthetist ward rounds of high risk post-operative patients.

 

Non theatre: We provide remote anaesthesia in a variety of locations (CT, MRI and ECT) around the hospital for adult and paediatric patients.

 

Obstetrics: The labour ward has approximately 3500 deliveries per year, with about a 25% Caesarean section rate and 20% epidural providing ample opportunity for clinical experience.

Intensive Care Medicine

ICM: Critical Care Services at York Hospital consist of a 17-bed unit with the bed-spaces used flexibly between ICU and HDU according to caseload. Currently there is nurse staffing to allow a maximum of 12 level 3 equivalent patients. There are approximately 1000 admissions each year.

There is also a well-developed critical care outreach service providing twenty-four hour support for ‘at risk’ patients throughout the hospital. This is nurse-led with critical care medical support.

There are currently ten critical care consultants providing 'week-at-a-time' daytime cover and fourteen consultants on a rota providing exclusive out of hours cover to critical care. The combined unit has around 1200 admissions per year (over 400 at Level 3) and offers the full range of non-tertiary treatments, including renal support. There has been an additional ICU workload generated from the North Yorkshire centralisation of maxillo-facial surgery, vascular surgery and renal medicine services on the York site.

There are two FICE mentors in the department as well as expertise in trans-oesopageal echocardiography.

Pain

Pain: York had one of the first Acute Pain Teams in the country and this is well embedded in our perioperative care. We have 4 consultants specialising in chronic pain and provide a comprehensive chronic pain service.

 

Audit, QIP and Research

Audit projects are actively encouraged and supported. Attendance at Clinical Governance sessions is also encouraged and provides a forum for presentation and dissemination of audit results.

  • We have an ongoing research programme, particularly involving high risk surgery. Many of our trainees have opportunities to get involved with this.
  • Teaching experience is provided and trainees are encouraged to get involved in our educational activities both locally and regionally.

Educational Activities

In addition to the regional exam related and novice courses we offer the following based at York

  • Every Tues and Wed 8 – 8.30 Anaesthetic and ICU teaching for all grades of trainee.
  • All Day Teaching 10 per year for all grades of trainee
  • Journal club for the whole department followed by a social at the local curry house.
  • Clinical Governance meetings which occur 10 times a year.
  • Simulation. We have a fully equipped anaesthetic Simulation room embedded in theatres. This is used extensively for in house training and regional courses.

Out of Hours

We run a 1 in 8 out of hour rota for the trainees with 13 hour shift (including an hour handover time at each end of the day). There are 2 trainees on at a time supported by 2 consultants (General/ Obstetrics and ICM).

To provide a high level of both patient care and support for trainees, there is a significant Consultant presence out of hours. The ICM Consultants are resident until 21:00 during the week and 17:00 at the weekend. The General/ Obstetric are resident until midnight and overnight if the skill mix of the trainees deems this necessary. 

Advanced Training Opportunites: York hospital offers advanced training in Vascular Anaesthesia, Obstetric Anaesthesia, Regional Anaesthesia and in Peri-operative care.

York is a beautiful city in a beautiful county. Whether you wish to walk our historical walls, climb to the top of the Minster or cycle across the Moors or undiscovered Wolds there will be something to excite you. We also have 2 theatres, an arts cinema, real ale pubs with ghosts and wine bars with style.

Commuting
Not everyone can “up sticks” to live here. York is very well connected by train (1h 50 to London) and a cyclist’s dream. York Trust is particularly supportive of cyclists with an excellent covered, secure cycle park and various schemes for buying bikes and equipment cheaply. You can “park and ride” or “park and cycle” to avoid traffic.

The General Infirmary is in the centre of the city, placed between the main shopping and commerce areas and the University. As part of the larger LTHT, where clinical services are constantly moving between sites, akin to Brownian motion, the LGI is home to Neurosurgery, Cardiac surgery, Paediatric surgery, Vascular surgery with all the attendant radiology and medical diagnostic services. Leeds is currently expected to become a level 1 Major trauma centre. In the department of anaesthesia we currently have 87 Consultants, 2 Associate specialists, 2 Staff grades and 48 Trainees. The office accommodation has 4 secretaries, a seminar room with Smart board, a sitting room, a trainee computer room and a television in a box awaiting an antenna.

Trainees

As a general rule, ST3 and ST6 years will be in the LGI, allowing the trainee to complete the spiral learning as described by the 2010 curriculum. There will also be a smattering of ST4/5 who may still need to complete individual modules and ST7 who are undertaking a specific Advanced module available in the LGI.

 

St James's Hospital (aka Jimmy’s)

'Jimmy’s' and the Leeds General Infirmary are each among the largest teaching hospitals in Northern Europe. They are both part of the Leeds Teaching Hospitals NHS Trust which itself is one of the largest Trusts in the UK. 
Jimmy’s, just North East of the city centre, started out as a workhouse for the poor in 1845 but by the end of the 19th century all the component buildings had become used for medical care of the poor. St James’ Hospital is named after Dr James Allen, a former medical superintendent and Sir James Ford who oversaw the conversion from workhouse to hospital. Some of our trainees still see Jimmy’s as a workhouse!

Jimmy’s has hit the limelight over the years for various reasons; the Yorkshire TV documentary “Jimmy’s” televised between 1987-1994 followed the fate of junior medical staff, and more recently for performing the first NHS live donor liver transplant in 2007.

From an Anaesthetic perspective the Department has roughly 45 Consultants and 36 trainees (predominantly ST’s with 4 or 5 CT1’s at any one time). We provide Anaesthesia for a wide variety of adult surgical specialities including transplants (livers and kidneys), Thoracics, Upper and Lower GI Surgery, Bariatrics, major Urology and Gynae Surgery to name but a few. There has been huge investment in and centralisation of cancer services on the St James’ site with the building of the state of the art Bexley wing. Being a tertiary referral centre, there is no shortage of challenging cases to test your clinical skills and boost your logbook.

There are approximately 34 critical care beds (16 level 3), the majority of which are looked after by Anaesthesia. Trainees allocated to ICU will be on a separate on call rota and disappear from Anaesthesia usually for 3 months at a time. There are 3 trainees on the ground at any one time in ICU (sometimes more during the day) with 2 consultants and between existing patients, outreach and ward/A+E referrals there is plenty of work to go round.

The Delivery Suite has just under 4000 deliveries per annum with an epidural rate approaching 50%. As a tertiary referral centre with vascular X ray many complex pregnancies (eg placenta acretia) are managed at Jimmies. The complex cardiac cases are referred to the Leeds General Infirmary where there is another obstetric unit of similar dimensions. Plans are afoot to merge the 2 units onto one “superunit”.

In summary Jimmies offers excellent anaesthetic experience in a friendly department together with sub specialist training in thoracics, transplants, obstetrics, chronic pain, intensive care medicine, vascular access and major abdominal surgery. Other core modules are available at the Leeds General Infirmary.

The hospital is minutes away from the M62 and M1 and is within easy reach of Leeds. It is about 20 minutes walk, or a short bus ride, away from Wakefield Westgate railway station.

Clinical Experience

The hospital is a new build large district general hospital, opened in February 2011. The anaesthetic department has expanded dramatically over the last few years. We now have 42 consultants, all with special interests that they wish to share with trainee anaesthetists. Training is particularly strong in difficult airway management and in anaesthesia for plastic and orthopaedic surgery under regional blocks. We have a regional adult burns centre with two burns intensive care beds running alongside our large consultant led critical care unit. (The exact number of beds that this provides seems to be a mystery but is probably round about 15).

All trainees have dedicated educational supervisors. We are able to deliver all the current curricular basic training blocks and a large number of intermediate and advanced training blocks. 
Intermediate Blocks we can deliver: General Duties, Intensive Care, Paediatrics, Pain, Ophthalmic, Plastics/Burns, and Vascular.
The educational leads aim to be approachable and contactable and we are very happy to be asked questions about training by current or prospective anaesthetic doctors. 

The canteen has a good range and is open daily. Car parking can be a problem but if you get there in time for morning lists to start then there shouldn’t be an issue. Trainees will also be allocated to some lists at Pontefract or Dewsbury Hospital.  Lists at Pontefract are mainly day surgery and give trainees the opportunity to learn more about this increasingly important sub-specialty. Lists at Dewsbury include both major surgery and day case surgery.

Rota

There are currently five fully working time regulation compliant rotas at Pinderfields. First on acutes is normally staffed by the least experienced doctor and covers acute theatres supported by the second on, who is the most experienced on-site doctor. The second-on also provides support to the critical care and obstetric anaesthesia resident doctors who provide the services that you may be able to deduce from their titles. There is also a novice on call rota on ICU and a second on Obstetric rota.

There are two dedicated on-call consultant anaesthetists available at all times, one covering critical care, the other covering acute anaesthesia and obstetric anaesthesia.

Teaching

There is a new comprehensive teaching package for novice anaesthetists.

Weekly teaching takes place on a Wednesday AM. There are a number of interesting departmental meetings and teaching sessions that trainees are welcome to attend including; intensive care evening meetings and morbidity and mortality monthly meetings,  a regional anaesthesia group that meet to demonstrate and practice regional blocks, a journal club, and of course the monthly departmental audit meeting. There is an excellent postgraduate centre with full library facilities
