Registration
All trainees must register with the Postgraduate Dean at the time they commence their training programme within Health Education England Yorkshire and the Humber (HEE YH). To register trainees are required to download and complete the Form R Part A (Trainee registration for Postgraduate Specialty Training). Trainees cannot be registered on the Postgraduate Dean’s database or issued with a training number until this form is received.
To register trainees are also required to indicate formally that they accept the conditions for taking up a training post by return a signed copy of the Conditions of Joining a Specialty Training Programme.
Training numbers will be issued (by email) to trainees after the start date of their training programme, provided they have registered by returning a completed Form R Part A. Trainees appointed to a run-through or higher training programme will be issued with a National Training Number (NTN). Trainees appointed to a core training programme will be issued with a Deanary Reference Number (DRN). HEE YH will maintain a database of all registered trainees and their achievement of competencies.

Please download and complete a copy of the Form R Part A from the revalidation webpage and attach a passport sized colour photograph,

and download and complete a copy of the Conditions of Joining a Specialty Training Programme document (linked below),

and return both documents to the Programme Support team for your School.
Please note due to the large volume of Form Rs received, the team will not be able to acknowledge receipt of the Form R.
Form R Part B
All trainees are responsible for maintaining their registration with the Postgraduate Dean by completing the Form R Part B on an annual basis. Trainees will be asked to complete the Form R Part B ahead of the Annual Review of Competence Progression (ARCP).
The Form R Part B includes declarations required for revalidation. Revalidation is the process by which licensed doctors are required to demonstrate on a regular basis that they are up to date and fit to practise.
