Potential projects:

  • Evaluation of care home ACP support mechanisms, demonstrating admission avoidance/reduction & early discharge
  • Development of virtual wards at home & removing barriers to domiciliary remote assessment by ACPs
  • Review of social care staffing levels & provision, identifying limitations on discharge planning, reduction of readmissions & care package initiation
  • Identifying opportunities of advanced clinic practice across the maternity system, to develop midwifery advanced practice & development of physician associates in the maternity workforce
  • Preliminary estimates of ACP workforce need across the ICS to optimise discharges, rehabilitation, admission prevention for frailty & end of life care
  • Optimise training placement opportunities for workforce development, identifying obstacles to having more trainees across the system, with a view to providing solutions to training bottle necks across disciplines such as: undergraduate nursing; undergraduate medicine, physician associate studies, & ACP programmes.

The leadership fellow will work with the SY faculty of ACP on behalf of the ICS to identify priorities for these workforce developments & present solutions that can be implemented.
